Night Out Participation Application

This form must be completed and submitted to the Community Policing Unit.  It may be sent via fax (724-8313), US Mail, scanned to email (wcook@lowerpaxton-pa.gov) or delivered in person to the police station.  It is permissible to use additional sheets or to write on the back of the form if more space is needed.
