Regulations for Reserved Handicap Parking Spaces

If a handicap parking space is approved, the following regulations apply:

• Any vehicle displaying a valid handicap placard, handicap registration plate, or disabled veteran registration plate may use any designated handicap parking space.
• All approved handicap parking spaces are subject to annual review.
• The privilege of a reserved handicap parking space may be immediately revoked in cases of misuse or abuse. 

Application Procedure

1. Complete the handicap parking space application form. FORM LINK BELOW
2. Have the Physician's Statement completed by your healthcare provider.
3. Obtain a valid handicap placard or handicap registration plate before submitting your application.
4. Submit the completed application packet to:

Important Information

The application packet consists of five (5) sections, a signature page, and a Physician's Statement.  Please complete all forms in their entirety and return the entire packet together. Doing so will help ensure your application can be reviewed as quickly as possible.
