I hereby state that I am at least twenty-one (21) years of age. I understand that I am responsible for the actions of the guests and their abidance to the applicable Ordinances of the Borough of Wyomissing incorporated hereto my reference. I hereby further agree to indemnify and hold harmless the Borough of Wyomissing and its directors, officers, employees, agents and their representatives, heirs, successors, and assigns, of and from any and all claims, causes of action, losses, injuries, damages, demands, judgement, penalties, and the like, including but not limited to, attorneys' fees and expenses incurred by the Borough in connection therewith, arising out of the Block Party referenced above. I further understand that under penalties of 18 Pa.C.S.A. §4909 relating to unsworn falsification that the information set forth above is true and correct to the best of my knowledge, information, and belief.

Borough of Wyomissing Ordinance Chapter 15 Motor Vehicles and Traffic Part 1 General Regulations Section 107 Use of Streets for Processions, Assemblages, Parades and Events Subsection 21 Block Parties 1. EVENT MAY NOT COMMENCE PRIOR TO 8:00 A.M. PREVAILING TIME 2. EVENT MUST END NO LATER THAN 10:00 P.M. PREVAILING TIME 3. NO ALCOHOLIC BEVERAGES SHALL BE PERMITTED IN THE STREET 4. STREET SHALL BE CLEARED OF MAJOR DEBRIS AFTER EVENT 5. OUTSIDE VENDORS ARE PROHIBITED UNLESS APPROVED BY THE CODES DEPARTMENT OF THE BOROUGH 6. PROCEDURES TO BE ESTABLISHED TO LIMIT VEHICULAR TRAFFIC ON THE STREET
