PA License Plate Replacement Form: Obtaining a Free Replacement with a Police Officer's Signature

If your Pennsylvania license plate has been stolen, damaged, or lost, you can obtain a free replacement by following these steps:

Obtaining the Form:

You can download the form at the following link: https://www.pa.gov/content/dam/copapwp-pagov/en/penndot/documents/public...  You can also obtain the form from our website or from the Penndot website. 

Obtaining a Police Officer's Signature:

Take the completed form to your local police department and have a police officer sign the form stating that the license plate needs replaced due to damage or peeling.

Submitting the Form:

Mail the completed and signed form to:

Bureau of Motor Vehicles • P.O. Box 68593 • Harrisburg, PA 17106-8593

Once processed, you will receive a new license plate free of charge.

Remember to keep a copy of the completed form for your records.
